Visit Information for Patients


If you are an established patient, please call (614) 533-6800 to schedule an appointment with our physician. Due to our large practice size, we are unable to accept new patients at this time.


Patients who have changes in address, telephone number or insurance are asked to arrive 15 minutes prior to their appointment times, in order to make changes to your record in our electronic medical record system.


Please bring your insurance card and co-pay to each office visit.
 

  • We require co-pays be paid at check-in, prior to your visit.

  • We accept all major commercial insurance plans and Medicare.

  • Please contact your insurance company to verify we are on your particular plan.

  • If you have Medicare, please bring your pharmacy insurance drug card.


Please bring ALL medication BOTTLES, inhalers, insulin, over-the-counter medications and glucose meter, if you’re diabetic.


Please have someone accompany the patient if he or she is not able to fully understand and carry out medical recommendations, or if the patient is unable to communicate clearly to our staff.


If you’re having a physical exam and/or coming in for cholesterol or glucose testing, you’ll be required to fast for 12 hours.
 

  • Water, BLACK coffee and medications are okay while fasting

  • No insulin when fasting


If you have a medical emergency, you should call 9-1-1. If you have an urgent problem, you should call our office immediately at (614) 533-6800.  


If you are a new patient, please complete this New Patient Registration Form and bring it with you to your first visit.
